Because most tumors are made up of lots of different cell types, not just “cancer cells”. This is called “tumor heterogeneity”.
For example, pancreatic cancer only contains 5-15% actual cancer cells; the rest being immune cells, fibroblasts, blood vessels, and other cell types.
This also affects other indications, such as bladder cancer, triple-negative breast cancer, non-small-cell lung cancer, and many more.
